Pharmacodynamics

Xenical is a specific, potent and reversible inhibitor of gastrointestinal lipases, characterized by a prolonged effect. Its therapeutic effect is carried out in the lumen of the small intestine and stomach and consists in the formation of a covalent bond with the active serine site of pancreatic and gastric lipases. At the same time, the inactivated enzyme loses the ability to break down fats, supplied with food in the form of triglycerides, into monoglycerides and absorbed free fatty acids. Since triglycerides that are not subjected to destruction in the body are not absorbed, fewer calories enter the body, which leads to a decrease in body weight. In addition, the therapeutic effect of Xenical is realized without the entry of its components into the systemic circulation.

Data on the fat content in the faeces suggests that orlistat begins to act 24–48 hours after ingestion. The abolition of the drug leads to a decrease in the concentration of fat in the faeces to the level recorded before the start of treatment after 48–72 hours.

Clinical studies of patients taking Xenical, prove that they have a more significant loss of body weight compared with patients who are prescribed diet therapy. The decrease in body weight was noted already during the first 2 weeks after the start of therapy and lasted 6–12 months even in patients who had a negative reaction to diet therapy. For two years, a statistically significant improvement in the profile of metabolic risk factors accompanying obesity was recorded. Also, compared with placebo, there was a significant decrease in body fat in the body.

The use of orlistat helps to prevent repeated weight gain. An increase in body weight, which constitutes no more than 25% of the weight lost, was noted in approximately 50% of patients, while the rest maintained body weight, which was reached at the time of termination of therapy (sometimes even a further decrease was detected).

Clinical studies, which lasted from 6 months to 1 year, convincingly proved that in patients with increased body weight or obesity and type 2 diabetes who took Xenical, their body weight decreases more significantly than in patients for whom only diet therapy was prescribed as a treatment . Weight loss occurred mainly due to a decrease in body fat. Prior to the study, even in patients taking hypoglycemic drugs, glycemic control was insufficient. However, with orlistat treatment, a clinically and statistically significant improvement in glycemic control was achieved. The therapy also led to a decrease in the concentration of insulin, a reduction in the doses of hypoglycemic drugs and a decrease in insulin resistance.

The results of studies conducted over 4 years confirm that orlistat significantly reduces the risk of developing type 2 diabetes mellitus (approximately 37% compared with placebo). The degree of reduction in the probability of occurrence of the disease was even more significant in patients with an initial violation of glucose tolerance (approximately 45%).

A clinical study that lasted 1 year and was conducted in a group of pubertal patients suffering from obesity, clearly demonstrated a decrease in body mass index in adolescents taking orlistat, compared with those who received only placebo. Also in patients taking Xenical, there was a decrease in fat mass and thigh circumference and waist and a significant decrease in diastolic blood pressure compared with the placebo group.

Pharmacokinetics

In patients with both obesity and normal body weight, Xenical is systemically affected to a minimum. A single oral dose of 360 mg does not lead to the appearance of unchanged orlistat in plasma, which indicates that its concentration does not reach the level of 5 ng / ml.

The volume of distribution of orlistat is almost impossible to determine due to poor absorption. In vitro, more than 99% of the compound binds to plasma proteins (mainly albumin and lipoproteins). A small amount of orlistat can penetrate the erythrocyte membrane.

Metabolism of orlistat occurs predominantly in the intestinal wall. Experiments have shown that approximately 42% of the minimum fraction of Xenical that undergoes systemic absorption consists of two main metabolites: M1 (a four-membered hydrolyzed lactone ring) and M3 (M1 with a split N-formle leucine segment).

The composition of the molecules M1 and M3 is an open β-lactone ring, they also very slightly inhibit lipase (1000 and 2500 times weaker than orlistat, respectively). These metabolites are considered pharmacologically inactive due to low inhibitory activity and minimal plasma concentrations (approximately 26 ng / ml and 108 ng / ml, respectively) when taking Xenical in small doses.

The main way of elimination involves the removal of non-absorbed orlistat with feces. About 97% of the received dose of Xenical is excreted in feces, and about 83% is excreted unchanged. The total renal excretion of all substances whose structure is related to orlistat is less than 2% of the dose ingested. The period of complete elimination of the drug from the body (with urine and feces) is 3-5 days. The ratio of the ways of excretion of the active component of Xenical in people with normal body weight and patients with obesity was the same. Orlistat and its metabolites M1 and M3 can also be excreted in the bile. Their plasma concentrations in the treatment of children do not differ from those in adult patients while taking the same dose of the drug. The daily excretion of fat from the feces during treatment with Xenical was 27% when taking the drug with food and 7% when taking a placebo.

Preclinical data and animal studies have revealed no additional risks for patients regarding the safety profile, toxicity, reproductive toxicity, genotoxicity, and carcinogenicity. Also, the teratogenic effect in animals has not been proven, which makes it unlikely in humans.

Indications for use

The use of Xenical is shown in combination with a moderately low-calorie diet for long-term treatment of obesity or overweight, including in patients with risk factors similar to obesity.

The drug is prescribed to patients with overweight or obesity in the treatment of type 2 diabetes in combination with hypoglycemic agents: insulin, metformin, sulfonylurea derivatives, or a moderately low-calorie diet.

Contraindications

  • Cholestasis
  • Chronic malabsorption syndrome,
  • Period of pregnancy and breastfeeding,
  • Hypersensitivity to the drug.

The safety and efficacy of the drug in patients with impaired renal function and liver, elderly patients and children under 12 years of age have not been studied.

Instructions for use of Xenical: method and dosage

Capsules are ingested, during or immediately (within 1 hour) after meals.

Recommended dosage: 1 capsule 3 times a day, during each main meal.

If the food does not contain fats or the patient skips breakfast, dinner or lunch, then the daily dose of the drug is reduced by the number of missed meals.

Balanced moderately low-calorie diet of the patient should contain up to 30% fat. The daily rate of calories, consisting of fat, protein and carbohydrates should be divided into three main methods.

Side effects

In clinical studies of Xenical, the following adverse reactions have occurred:

  • On the part of the gastrointestinal tract: very often - a strong urge to defecate, discharge from the rectum oily structure, steatorrhea, gas with a small discharge, increased feces, loose stools, discomfort or abdominal pain, flatulence (frequency increases with increasing fat in food), often - bloating, soft stools, fecal incontinence, pain or discomfort in the rectum, damage to the teeth and / or gums,
  • Other: very often - headache, upper respiratory tract infections, flu, often - weakness, dysmenorrhea, anxiety, urinary and lower respiratory tract infections, in patients with type 2 diabetes - hypoglycemic states.

In the postmarketing observations described the possible cases of side effects:

  • Allergic reactions: rarely - itching, skin rash, bronchospasm, urticaria, anaphylaxis, angioedema, very rarely - bullous rash,
  • Other: very rarely - increased alkaline phosphatase and transaminase activity, hepatitis, rectal bleeding, diverticulitis, pancreatitis, cholelithiasis and oxalate nephropathy (the frequency of occurrence is unknown).

Overdose

Clinical studies in which people with normal body weight and obese patients participated, who took a single dose of 800 mg or were treated with Xenical for 15 days and received it in a dosage of 400 mg 3 times a day, do not confirm the occurrence of significant adverse events. Also, patients taking orlistat 240 mg 3 times a day for 6 months did not experience significant health problems.

Thus, in case of an overdose of Xenical, adverse effects are either absent or similar to those registered with the use of the drug in therapeutic doses. In cases of severe drug overdose, monitoring of the patient’s condition for 24 hours is recommended. According to animal and human studies, all systemic effects associated with the lipase-inhibiting properties of orlistat are quickly reversible.

Special instructions

According to the instructions, Xenical with long-term use allows you to control the reduction and maintenance of body weight at a new level, preventing re-gaining extra pounds.

Exceeding the recommended dose of orlistat does not enhance its therapeutic effect.

The clinical effect of the drug reduces visceral fat and improves the profile of risk factors and pathologies associated with obesity, including impaired glucose tolerance, type 2 diabetes, hyperinsulinemia, hypercholesterolemia, and arterial hypertension.

Simultaneous use of the drug with hypoglycemic agents (sulfonylurea derivatives, metformin, insulin) and compliance with a moderately low-calorie diet allows patients with type 2 diabetes with obesity or overweight to further improve the compensation of carbohydrate metabolism.

In the majority of patients after four years of using orlistat, clinical studies confirm that beta carotene and vitamins A, D, E, K are within the normal range. To provide the body with an adequate supply of nutrients, the administration of multivitamins is indicated.

A moderately low-calorie diet should be balanced, contain a lot of fruits and vegetables, and 30% or less calories in the form of fat. The daily rate of carbohydrates, fats and proteins should be eaten in three main steps.

The likelihood of side effects of the drug from the gastrointestinal tract is increased against the background of high-fat foods.

The use of Xenical in diabetes mellitus type 2 improves the compensation of carbohydrate metabolism and may cause the need to reduce the dose of hypoglycemic agents.

Use during pregnancy and lactation

Reproductive toxicity studies conducted in animals did not reveal the teratogenic and embryotoxic effects of Xenical. It is assumed that the drug is safe for pregnant women, but due to the lack of clinically confirmed data, its use during this period is not recommended. It is not known for sure whether orlistat penetrates into breast milk, therefore during the treatment it is necessary to stop breastfeeding.

Drug interactions

There was no clinical interaction of Xenical with the simultaneous use of amitriptyline, atorvastatin, biguanides, digoxin, fibrates, fluoxetine, losartan, phenytoin, or other people, but in any case, in any case, in any other case, in any case, in any case, in any case, in order to apply, or if they are applied to the gastrointestinal therapy, or if they are in a diet. However, when combined with oral anticoagulants, including warfarin, it is recommended to monitor the indicators of international normalized ratio (INR).

There is a decrease in the absorption of beta-carotene and vitamins D, E, so multivitamins should be taken at bedtime or 2 hours after taking the drug.

The combination with cyclosporine can lead to a decrease in its concentration in the blood plasma, therefore, it is necessary to regularly determine the plasma content of cyclosporine when combined with orlistat.

Due to the lack of results of pharmacokinetic studies, the simultaneous use of acarbose is contraindicated.

Against the background of simultaneous administration of Xenical and antiepileptic drugs, cases of the development of seizures in a patient were recorded. Since the causal relationship of this interaction has not been established, the frequency and / or severity of the convulsive syndrome in this category of patients should be monitored.

Analogs of Xenical are: Xenalten, Orsoten, Orsoten Slim, Orlistat Kanon, Alli, Orlimax.

Reviews of Xenical

According to reviews, Xenical causes an ambiguous attitude in patients. Most of them argue that its application will be effective only in the case of a comprehensive struggle with the problem of excess weight.

Many doctors believe the drug is a good help in the treatment of obesity, but its reception must be combined with a low-fat diet. During 1 month of Xenical treatment, even without significant physical and physical stress, you can lose 1.5-2 kg. Even better results are achieved when combining similar drug therapy with sports.

Depending on the individual characteristics of the body and the careful observance of the doctor’s prescriptions, it is possible to reduce body weight by 10-15 kg in 3 months, and by 30 kg in 6 months.

Use during pregnancy and lactation

Category B drug.

In animal reproductive toxicity studies, no teratogenic and embryotoxic effect was observed. In the absence of a teratogenic effect in animals, one should not expect such an effect in humans. However, due to the lack of clinical data, Xenical should not be given to pregnant women.

Excretion of orlistat with breast milk has not been studied, so it should not be taken during breastfeeding.

Interaction

No interaction found with amitriptyline, atorvastatin, biguanides, digoxin, fibrates, fluoxetine, losartan, phenytoin, oral contraceptives, phentermine, pravastatin, warfarin, nifedipine GITS (gastrointestinal therapeutic system) and nfedipinomi, sidroperoceroxinoloneocetricin, niadipineom GITS (gastrointestinal therapeutic system) and nfedipine, niadipineum GITS (gastrointestinal therapeutic system) and nfedipine, niadipine, GITS (a gastrointestinal medication) and nfedipine, niadipine, GITS (based on research on drug interaction). However, it is necessary to monitor the performance of MNO with concomitant therapy with warfarin or other oral anticoagulants.

When taken simultaneously with Xenical, there was a decrease in absorption vitamins A, D, E, K and beta carotene. If multivitamins are recommended, they should be taken at least 2 hours after taking Xenical or before bedtime.

While taking Xenical and cyclosporine a decrease in plasma concentrations of cyclosporine was noted, therefore, more frequent determination of plasma concentrations of cyclosporine is recommended, while taking cyclosporine and Xenical.

Upon appointment amiodarone during therapy with Xenical, there was a decrease in systemic exposure of amiodarone and dezethylamidarone (by 25–30%), however, due to the complex pharmacokinetics of amiodarone, the clinical significance of this phenomenon is not clear. Adding Xenical to long-term treatment with amiodarone may lead to a decrease in the therapeutic effect of amiodarone.

Simultaneous administration of Xenical and acarbose due to the lack of data pharmacokinetic studies.

Dosage and administration

Inside adults - 1 caps. (120 mg) with each main meal (during, or no later than 1 h after a meal). If a meal is missed or food does not contain fat, then the drug can also be skipped.

Increasing the dose of orlistat over the recommended (120 mg 3 times a day) does not lead to an increase in its therapeutic effect.

Dose adjustment in patients old age not required.

Dose adjustment in violation of the function of the liver or kidneys not required.

Safety and efficacy of Xenical in children under 18 not installed.

Post-marketing observations

To date, there are descriptions of cases of hypersensitivity reactions, the main clinical manifestations of which were hives, bronchospasm, itchskin rash angioedema, anaphylaxis.

In rare cases, increased activity of alkaline phosphatase and transaminases and bullous rash were recorded. Separate (probably serious) cases are also described. hepatitis against the background of drug treatment (neither the pathophysiological mechanisms of development nor the causal relationship with the use of Xenical was not established).

Use of the drug in combination with anticoagulants led to a decrease prothrombin and change hemostatic parameters.

Other side effects reported during post-marketing observations:

  • oxalic nephropathy,
  • diverticulitis,
  • cholelithiasis,
  • pancreatitis,
  • rectal bleeding.

When taking the drug with antiepileptic drugs there have been cases of convulsions.

Xenical tablets, instructions for use

Long-term treatment of patients with overweight / obesity (including patients with predisposing factors of obesity) is carried out with the use of 120 mg for each of the main meals. The capsule is taken on or during the hour (but not later!) After meals.

Patients with insulin dependent diabetes the drug is prescribed in the standard dosage - 1 capsule at each meal. If there is no fat in the consumed food or the patient misses a meal, Xenical can not be taken.

How to take a drug to lose weight?

As an adjunct to therapy, the patient must follow a balanced, low-calorie diet, in which calories in the form of fat do not exceed 30%. The daily dose of proteins, fats and carbohydrates should be divided into three main steps. In the diet should prevail vegetables and fruits.

The instructions for use of Xenical indicate that an increase in the recommended dose (360 mg / day) does not lead to an increase in its therapeutic effect.

Analogs of Xenical

Structural analogues of the drug are: Xenalten, Orlistat Canon,Allie, Orsoten, Orlimax, Orsoten slim.

The price of Xenical's analogues is from 404 rubles. The most inexpensive substitute of the drug is Orsoten Slim (60 mg capsules in packs No. 42 cost an average of 450 rubles).

The safety and effectiveness of Xenical in patients younger than twelve years of age has not been studied.

Xenical Diet Pills: Efficacy in Obese Patients

In clinical studies with host orlistatpatients had significant weight loss compared with patients who were prescribed diet therapy.

Body weight began to decrease already during the first two weeks of treatment and continued to decline over the next 6-12 months (including in patients with a negative response to diet treatment).

For 24 months, a statistically significant improvement in the metabolic risk factors associated with obesity was observed. Compared to placebo, the amount of fat in the body also decreased significantly.

Xenical effectively prevents re-gaining weight: weight gain, not exceeding ¼ of the loss, was observed in about 50% of patients, with half of the patients not regaining weight gain, and in some cases weight continued to decline.

Use of the drug for weight loss in patients with insulin-dependent diabetes

Studies lasting from six months to a year in patients with a BMI of more than 28 kg / m2 have shown that in patients receiving therapy orlistat, there is a significantly greater weight loss in comparison with patients who were prescribed only a therapeutic diet.

Weight loss occurs mainly due to a decrease in the percentage of body fat.

It must be emphasized that prior to the study, despite hypoglycemic agents, patients often had insufficient control. hypoglycemia.

However, during treatment with the drug, a clinically and statistically significant improvement in control was observed. hypoglycemiadecreasing insulin resistance, and also decrease in concentrationinsulin and patient needs hypoglycemic agents.

In addition, four-year clinical studies have shown that the use of Xenical can significantly (approximately 37% compared to placebo) reduce the risk of developing insulin dependent diabetes. At the same time, in patients with impaired glucose tolerance, the degree of risk reduction was even more significant (45%).

Maintaining weight and improving the profile of predisposing factors at a new level was noted throughout the entire study period.

The effectiveness of Xenical in pubertal obesity

In the group of obese adolescents, studies were conducted over a year. In all patients under observation, BMI decreased more pronounced than in patients taking placebo.

Patients taking the drug significantly reduced the percentage of body fat and diastolic pressure, and also significantly reduced the size of body circumferences (hips, waist) in comparison with patients in the control group who received a placebo.

During pregnancy

Category B drug.

During reproductive toxicity studies conducted on animals, no embryotoxic and teratogenic effects were detected. However, the fact that the drug did not cause embryonic development disorders in animals does not mean that this is not possible in humans.

Due to the lack of clinical data, the drug is not prescribed to pregnant women.

Allotment orlistat it has not been studied with breast milk, and therefore it is not recommended to take Xenical to lactating women.

5 reviews to “Xenical”

I lost 25 kg on Xenical for half a year, I had no side effects, I strictly followed the diet that the doctor wrote to me, and my weight loss went well

Your initial weight was how much?

From the priests flowed a whole month while taking pills. Not lost on a gram.

With this drug, it is very easy to lose teeth, hair and generally earn a lot of problems. Since the lipase blocker also takes care of not absorbing fat-soluble vitamins A, D, E ...

Xenical buy is not the first time. I want to say that the drug works very effectively, if of course you adhere to proper nutrition. Now bought it exclusively for those days when you overeat (holiday for example). Pure weight support is normal.

When the drug is prohibited

The drug also has a number of contraindications, the main ones include:

  • allergic to components that make up capsules
  • cholestatic syndrome
  • malabsorption syndrome.

During the period of childbearing and lactation, it is forbidden to take Xenical, since no clinical studies have been conducted showing how the remedy affects women in the position. It is also not known how it affects the embryo and breastfed babies.

Xenical or Orlistat: what to choose?

Very often, people suffering from obesity have a choice of which drug to drink - Xenical or Orlistat. When choosing between drugs, it is recommended to contact your doctor. It takes into account the individual characteristics of the body, chronic diseases, medications taken on a regular basis, and the like. Therefore, the doctor can determine which of the drugs is more suitable for the patient. Orlistat should be taken 3 times a day, 1 capsule 120 mg together with a meal. The adverse reactions of the body in both drugs are identical. But according to reviews losing weight, Orlistat has fewer side effects. And if they appear, then when applied for more than 15 days.

Orsoten or Xenical: comparative characteristics

The composition of the drug Xenical and Orsoten includes the same active ingredient Orlistat. Therefore, the actions of drugs are almost identical. For a long time, Xenical did not have any analogues and was quite an expensive drug, but in 2007 it had analogues. Orsoten is an analogue of the drug Xenical. The main differences are the color of the capsules and the difference in price. Orsoten is more affordable. The list of adverse reactions and contraindications for the same means. They are interchangeable.

Can I buy without a prescription
